Investigation of Unicare Insurance for Failure to Pay Benefits
We are investigating whether three UniCare insurance companies — UniCare Health Insurance Company of the Midwest, UniCare Life and Health Insurance Company, and UniCare Health Insurance Company of Texas — failed to pay health insurance benefits provided in their policies. These UniCare insurance companies are subsidiaries of WellPoint, Inc.
The Benefits.
We are investigating whether these UniCare insurance companies failed to pay full benefits for infusion therapy. Infusion therapy includes:
| -- chemotherapy | -- nutrition via feeding tube |
| -- antibiotics | -- tocolytics (to stop premature labor) |
| -- pain management | -- aerosol therapy |
| -- total parenteral nutrition (TPN) | -- blood transfusions |
| -- intravenous hydration (IVs) |
Our investigation is on behalf of people who received infusion therapy as inpatients or as outpatients. People receive infusion therapy on an outpatient basis at hospitals, physicians’ offices, skilled nursing facilities and from home healthcare providers. Most people who are hospitalized receive at least one form of infusion therapy, such as IVs or injected pain medication.
UniCare Policies for Both Families and Employees Have These Infusion Therapy Benefits.
-- If you have individual or family health insurance from one of these
UniCare companies you may have a claim;
-- If you are an employee of a company that has health insurance from
one of these UniCare companies you may have a claim under the
Employee Retirement Income Security Act (“ERISA”).

Illinois, Arkansas, Georgia, Indiana, Nevada, Texas and Virginia.

August 1, 1998 to the present date.
